Trip to Kyushu or Hokkaido with mom who struggles with walking – car rental?

Hi guys! I’m planning a 2-week trip to Japan, possibly Kyushu or Hokkaido, with my family. We’ve been to the big cities (Tokyo, Osaka, etc) many times and we are all interested in somewhere new. We also like the idea of having day trips into the countryside. I have a couple questions for those of you more Japan-savvy than me:

1. 1. Would you all recommend Kyushu or Hokkaido for an October trip? Last year I went to Tokyo in October and it was still quite warm–I’m afraid that Kyushu will be too hot for us San Franciscans who are used to foggy weather. Hokkaido seems like a great alternative. Any thoughts? (If you need more info about our interests – brother and his gf are in the culinary industry and love trying new foods; mom loves taking photos of scenic places; my bf and I are into games and anime, I love Japanese fashion)
2. My mom has some chronic leg pain and cannot walk for too long of a time. Would it be in our interest in renting a car for our trip? I have no idea how to get a international driving permit, and I’m worried about getting into an accident since we are used to US driving customs (and of course driving on the right side!) Also, how expensive is renting a car? We don’t mind paying a premium to let my mom enjoy the trip but just wondering.
