I searched online and found this form from VN Embassy [https://www.vn.emb-japan.go.jp/files/000475049.pdf](https://www.vn.emb-japan.go.jp/files/000475049.pdf) and this one works, as in QR not showing in browser PDF reader but it appeared when opened in Acrobat. But it has a different photo dimension so I can’t use it.
Anyone can help?
