1. You specify an exact ordinal with 目 (め).

   ５回 – Five times

   ５回目 – The fifth time

   Same thing with ページ

2. I think it can mean both but “read page 7” is the more natural interpretation in most contexts

4. Could you distinguish it as 7ページを読んでください (read page 7) or ページを7枚読んでください (read 7 pages)？

5. read 7 pages = 7ページ分を読んでください

6. 7ページ読みなさい。
   7ページ目を読みなさい。
