You can get that at any station. Childrens’ Suica, commuter passes, etc. never had suspended sales.
We ended up going to the JR EAST Travel Service Center in Shibuya. I just brought my child’s passport, and they filled out a quick form and issued one. It was about a 5 min wait in line on a Sunday afternoon.
