Pension lump-sum document confusion

Hello,

Leaving Japan in April, and I want to apply for my lump-sum pension payment once I’m home. However, the information online seems to be contradictory and I could use some clarity.

My understanding is that I need:

1. The application form itself.

2. A copy of my passport (the page(s) showing your Status of Residence and the date of my final departure from Japan, as well as the page(s) showing my name, date of birth, nationality, and signature).

3. A copy of my bank statement, bank passbook, etc. which verifies my bank’s name, branch office name, branch address, bank account number, and the full name of the account holder.

4. Pension Book.

#2 is confusing me. Where in my passport would it show my “status of residence”?

My wife and I did our moving out procedure today at the city hall, however they did not give us any documents afterwards. How can I prove that I no longer have residence in Japan?

(My wife is not Japanese so she doesn’t know the procedure strongly either.)

Sorry if I’m coming across like a complete moron, my brain is a bit fried from the move and I’m worried I’m missing some important thing to get my pension pay out.
