Spouse Visa: US Remote Worker. Live less than 6 months a year in Japan but have an apartment in my name

Sorry long post. I have a somewhat complicated question. Need some thoughts about where we should go to figure out our taxes.

TL;DR as title says work in the US remotely in IT. Spouse Visa. Stay less than 6 months in Japan a year. Have an apartment in my name and signed up for local healthcare for a bad cold last year. Kansai area, 所得申告書 received. Need to understand options.

Summary
2022: 3 months in Japan РFianc̩e Visa

Engaged beginning of 2022. Married October in Japan. Spent marriage and honeymoon time in Japan.

2023 – 1 year Residency card- 5 months in Japan, no property

We have been staying in different countries while I work because the US green card process takes while. During the winter of 2023, got bronchitis, so the wife enrolled me into the healthcare. Apartment in wife’s name.

2024 – 3 year Residency card- 1 month so far in Japan, moved to new apartment in my name. Plan on 5 months in Japan again this year. Apartment in America as well.

Feb 2024, got an apartment in my name because wife has been traveling the world with me but not working (she is a freelancer).

We got a letter from local municipality 所得申告書 asking what I do for work. Basically they want to know how much they have to charge for my healthcare. My options are Salary, retirement, none, or other.

I think should choose other or none.

My thoughts: I work for a foreign company and I’m not in Japan most of the year. A tax resident and visa residency are different.

I don’t mind paying taxes I just need to figure out do I need to be double taxed for America and Japan or are my thoughts correct.

2023 made 160k
2024 probably around the same

I thought if I lived in Japan under 6 months a year, foreign income is not taxed.

It would only be remittance. But the only money that I send is for rent monthly, wife pays everything else.
