I’m in the process of applying for my child’s UK passport (she is a japanese national but registered with the UK authorities and has a UK birth certificate).
I have been scrutinising similar posts on this forum which have been very helpful and have come up with the following list. If anybody has recently gone through the process and could let me know if I am missing anything I would be most grateful.

Full local birth certificate (shussetodoke kisaijiko shomeisho). Issued by the Japanese authorities.
Her Koseki
Hospital birth certificate
My birth certificate
My UK parents’ birth certificates
Proof of parent’s marriage (Japanese spouse koseki)
Consular Birth Certificate issued by UK
Proof of residence in Japan
Photocopy of my UK passport
birth registration name confirmation form ([https://www.gov.uk/government/publications/birth-registration-name-confirmation-form](https://www.gov.uk/government/publications/birth-registration-name-confirmation-form)) (This is for the UK passport and I have not filled this in when they were born)
2 x photos
testimonial of a UK reference who has known her since birth (is a doctor).
