My visa (Engineer/Specialist in Humanities/International Services) is expiring on October, So I am planning to apply for extension within this month. I have prepared the required documents from both mine and company’s side. I havent change a job since enrolled in the current job. My status is 正社員.But I am bit confused on some of the documents of its required or not.
Here are the documents I have prepared so far.
1)application form (filled out from mine side and company’s side)
2)Kazei Shomeisho
3)Nozei Shomeisho
4)Salary slip(of last three months)
5)Passport
6)Residence card
7)Photo
Company Provided following documents
1) 履歴事項全部証明書 (company’s record history?)
2) 在籍証明書 (enerollment certificate)
My question is , are these document enough?
Upon googling a bit, i also found we need to show health insurance payment. But my health insurance is not from city hall but instead company’s health insurance . Where can I get that payment record from?
Also do we need to show pension payment documents too?
My salary slip have all included like pension, insurance, tax.. as they are deducted every month from the salary, is it necessary to provide those (health insurance and pension) documents too?

You’ve got way more than I’ve ever supplied. If you need proof you’re paying health insurance, your tax documents show that. It’ll indicate money was taken from your salary for health insurance.
Also the nozei shomeisho has the kazei shomeisho information already on it. I double prepared these for years until someone at city hall looked at the immigration requirements webpage with me, and said oh yeah the nozei by itself is good enough 🙂
As far as I am aware health insurance and pension proof is only need for Permanent Resident application (personally I think they should be included for work visa extension/changes too, but that’s maybe just me?)
However, I’ve always fair OK with my renews (got extended onto 3 year work visas when on work visas) and a got a 3 year spouse visa on my first spouse visa renew (got the dreaded 1 year for the 1st spouse visa though). I’ve always just included a photo copy of my health insurance card (and blacked out the health insurance number).
This helps prove to immigration I am enrolled in Shakai Hoken and paying my health insurance and pension via my employer/salary every month. So I’d say no harm in doing that if you like.
