Itinerary feedback/help too ambitious

We’re going to Japan for 15 days, arrive May 28th at 6h30 am and leave on June 11th late in the evening. We’re spending the first 3 nights in Tokyo. after this is where things go sideways and we’re wondering if we’ve crammed too much in.

We are debating flying to KIX and setting up base in Osaka for 4 nights and doing day trips from there. Hiroshima one day, Miyajima and Himeji on others. Then going to Kyoto and a day trip to Nara before going back to Tokyo on the 7th or 8th and possibly doing a day trip to see Mount Fuji if it’s clear. The other option is flying to Hiroshima and making our way back to Tokyo, it will just be a lot of spending a night or two every place until Kyoto.

The thing is this will be my only trip to Japan due to terminal illness so I’m trying to see as much as I can without overdoing it. Is there anywhere that I should look into going or anywhere that is a skip? Maybe a day in Hakone? Interests are art, history, museums, like to see a baseball game and the ocean. We aren’t into theme parks and I’ve never gotten into anime.
