How to schedule a skin test with a doctor in Japan?

I am wanting to get a yearly skin test to check if spots are cancer or not.
Which doctor do I go to and what kind do test do I ask for?
Is this a normal procedure done in Japan or is it seen as troublesome?
Would it fall under normal health insurance or will it cost extra?
