What’s the deal with 固定金額変更連絡票 ?

Can anyone explain what these forms are and why I’m stuck doing them every month? My boss says we have to give these to the bank with a list of all our students and how much they all paid us, I don’t understand what for… His explanation doesn’t go beyond that and my coworkers can’t seem to explain it to me either, they just shrug and give me the “that’s how it is” look. And it all has to be hand written from the beginning every single time, I’m really sick of writing out all the client numbers and names etc in katakana and getting complaints that my ア looks too much like イ or ソ and other nitpicky stuff, which they don’t really… Besides, all these details are in an excel spreadsheet, I really don’t understand why we can’t just give them a printout or digital copy of the spreadsheet and be done with it, no doubt someone is then on the other end typing all this up into their own excel sheet… I have better stuff to do like make lesson plans.
