Nominating a tax representative after leaving Japan

I received my pension refund a few months ago and have finally got around to looking into how to receive the tax refund and just wanted to make sure I’m understanding the process currently.

I never chose a tax representative while I was still living there, so if anyone who’s gone through it themselves could let me know how it works it would be greatly appreciated, because I’ve been struggling to get my head around it!

So I should:

1. Fill out this form: https://www.nta.go.jp/taxes/tetsuzuki/shinsei/annai/shinkoku/pdf/07.pdf
2. Send it to my tax rep, along with the receipt for the receipt of my pension refund
3. They go to my local tax office, and submit the forms along with the tax return on my behalf
4. They receive the money a few months later and transfer it to me

I think the main thing I’m confused about is whether I have to fill in any tax return form, or if my tax representative just does that by themselves at the tax office – can’t seem to find a clear explanation online.
