Receiving 20% withheld tax from pension lumpsum: few questions

Hi,

I left JP last December and got the lump sum pension withdrawal amount in April. I have few questions about filing the return. I have already settled my Income tax and resident tax.
1. I appointed my friend as tax representative and I need to tell him what to do as he isn’t aware of this process.

3. I found this from the NTA website. This has extra pages compared to the above guide, should I just leave them out?

4. Do we need to send any other forms? Like the withholding slip I received back in my home country?
