Anyone recently filled out a kojin jigyo – 個人事業 form?

Hi all,

I’ll be going to the tax office to hand in the kojin jigyo form. Wondering if anyone has done this recently that would be happy for me to DM for some advice? Some of the form I don’t fully understand!
