24 day Japan itinerary with panic disorder (march/april 25)

Hi friends, I (27m), first timer, am planning my 24-day solo trip to Japan in march/April 25. I’d love to hear your opinion and advice on my itinerary. Is it way too overpacked? I’m really into ancient Japanese architecture, anime and tech.

I am visibly tattooed but would like to plan a Ryokan stay (preferably with onsen) along my route. Are these worth the higher prices in regions like Fuji/Kamakura/Nara?

In addition, I have been suffering from panic attacks for several years, which manifest themselves in situations when I am alone. I was and will go to therapy again soon to work on myself. I want to fulfill a lifelong dream with this trip and free myself from the chains of myself. My question is, how is the topic of mental illness in Japan, are they a taboo? And how would the locals react if help was needed in a panic situation, even though the language barrier?
