My wife is Japanese – do I still need Nomad visa to work?

My wife is Japanese and we have family that live in Japan. I work in the USA.

I would like to spend summers in Japan and work remotely so the Digital Nomad visa looks great. However as a spouse of a Japanese citizen are there better ways to go about this? Does being her spouse prove opportunities for health coverage for the Nomad visa or otherwise or do I just jump through the same hoops to get international insurance if some type that works in Japan.

Appreciate any feedback as I research a path.
