I Got a Municipal Tax Booklet in the Mail. Hurray…(what is this?)

I've never received Municipal Tax (市県民税) like this before.

I have 4 separate slips, one due July 1st, next September 2, then October 31st and last on January 31st.

I've never gotten a tax booklet for with 4 different periods (1-4期) and the amounts are so goddamn high.

Can anyone explain what's going on here?
