Has anyone used the お土産 免税 before?

Since I am about to leave for summer vacation, I saw that it is possible for long-term resident to purchase goods tax free if it is intended as a gift for someone abroad.

It seems that if you are to take the item out for 2 years/leave it outside of Japan, you are able to come back and be reimbursed for the sales tax you have originally paid.

However, after some digging around nothing have come up. Has anyone here used it before? If so, how straight forward was the process?

I see that both 輸出証明申請書 and 購入者誓約書 are required, should I prepare these or are these done at the store? I have never used any tax free services in Japan before so I am unaware of the normal procedure.
