Does anyone know where I get this. I am leaving the country for around 7weeks, I know I need to acquire this permit but I have seen two different sources stating different locations. One source says I have to go to immigration another says I just fill out a form at the airport.
Can someone clarify ?

You fill it out at the airport before passing through immigration. The form can be filled out at immigration, which is just after you go through security. That’s how it worked at Narita.
みなし再入国許可 is basically the embarkation card you fill out at the airport that gets stapled into your passport — perhaps depending on your country/status type YMMV
The [“special” reentry permit is the embarkation/disembarkation card](https://i.imgur.com/zLsOwKn.jpg) that you pick up and fill out at the airport and immigration staples into your passport. It’s valid for up to a year.
If you are planning to be out of the country more than a year, you need to go to immigration to get a *proper* reentry permit.
Also, this is all post-2012 changes to immigration, so any websites with information from before that will be confusing and wrong.
