I have PR in Japan and I plan to try and keep it for as long as I can.
I have family in the states as well though so I would like to meet them for extended periods before they pass away.
My question is, could I get a 5-year re-entry permit, change my residence to the US, and then come back to Japan before 5 years are up to just re-register a japan residence at a cheap share house for instance, stay for 3-6 months at least and then de-register my residence and leave Japan again with another 5 year re entry permit? Could I do this multiple times? ( I need to take care of my parents but I don't want to lose PR).

I’d check with an immigration lawyer/scrivener, but my guess would be that you’d only need a contact address to fill out any form, and you would not need to formally register residency.
A formal registration (eg, city hall) may trigger at least one year of a tax cycle, and maybe also follow-ons like health insurance and pension.

Possible in theory, but Immigration may question you on that 2nd time as to if you are really intending to resume your residency.
