Medical certificate (診断書) in English for travel

I have a prescription for a medicine considered a controlled substance in the US, and I have plans to travel there soon. Based on what I’ve read if I have a medical certificate I am allowed to carry them with me over the border, but the certificate must be in English. Is it difficult to get the certificate not in Japanese? Or will I have to get the certificate professionally translated? I get this prescription from a local small clinic in my town. I don’t have a lot of money, so if anyone has experience getting it in English directly from their doctor I’d like to know your experience. I can’t leave the medicine home since the condition is essentially chronic and pretty debilitating.
