Going to Japan for the first time with my wife in October. This is our current itinerary:
Day 1: Tokyo
Land in HND at night
Eat Dinner and chill
Day 2: Tokyo
Tsukiji Market
Tokyo Sky Tree
Explore Asakusa
Day 3: Tokyo -> Kawaguchiko
Eat breakfast and take train to Mt. Fuji
Stay at onsen in Lake Kawaguchiko
Naruto Village
Mt. Fuji Ropeway
Day 4: Mt Fuji -> Osaka
Take train to Osaka
Osaka Castle
Dotonbori at night
Day 5: Osaka
Day trip to Kobe and Hiroshima
Still not sure what areas in Kobe and Hiroshima to hit
Day 6: Osaka
Day trip to Kyoto
Fushimi Inari
Arashiyama Bamboo Forest
Day 7: Osaka -> Kusatsu
Take early morning plane back to Tokyo
Take train to Kusatsu
Stay in Kusatsu Onsen and explore area
Day 8: Kusatsu -> Tokyo
Take train back to Tokyo
Stay in Minato
Pokemon Center
TeamLab Borderless
Day 9: Tokyo
Parco Museum
Yoyogi Park
Shibuya Sky
The Making of Harry Potter
Day 10: Tokyo
Tokyo Disneyland
Day 11: Tokyo
Ginza
Open for anything else
Day 12: Tokyo
Breakfast
Take train to HND for morning flight

We are still not sure what specific things we want to do in Kobe and Hiroshima, as well as things we can do in Day 9 – 11 of our time in Tokyo.
