Brain death and organ transplants?

Hey folks, I just received my new insurance card in the mail and am deciding whether to donate organs or not, and if so, in which event to donate them (brain death or cardiac/circulatory death). In my home country I was always an organ donor, and I'd like to continue that here; I just wanted to read up on the specifics first.

I've been reading through the Japan Organ Transplant Network (JOTNW) website and am hung up on one specific point: It seems like brain death can only be officially (legally?) declared if the deceased person has arranged for organ donation in the event of brain death. If they have not arranged for such a donation, then they cannot be declared brain dead. Am I understanding this right? I've included the excerpt and link below.

「日本では、脳死での臓器提供を前提とした場合に限り、脳死は人の死とされます。」

I'm mainly hung up on it because if a person would have otherwise been declared brain dead if they were donating organs, but since they're not donating they're not considered brain dead, what then happens to the person? Is their family presented options different from if they were planning on donating? Thanks for any and all insight!
