I can’t remember specifically which restaurant I’ve had it at, but I’ve had it at nicer than avg. Italian restaurants. Dean and DeLuca usually has 1-2 types for take out at Tokyo station.
Wherever you find it be prepared to be underwhelmed by the portion size. I think I would have needed 3-4 portions to be full without other dishes (salad, meat, other pasta, etc.)…
I like the lasagna at dean and deluca. It’s a pretty big piece too.
If you can’t find a good restaurant, Picard has quite nice frozen lasagna. They have several branches throughout Tokyo.

Not traditional lasagna, but parabola in Yoyogi is a small bistro that serves a good “deconstructed” lasagna of sorts.
I’ve given up looking and have to make my own how Mother taught me.
Lasagna Bar in Kashiwa is run by a really fun Italian guy and the lasagna is incredible.
Best lasagna I have had is from Dean & Deluca.
Ikea. The meat-free lasagna there is fantastic. I had two portions last week.
I can’t personally speak to the quality of the place, as I’ve never been, but I have a friend who has been to this place in Koenji and says it was pretty good:
I just make it at home if I want it. You can get the noodles for cheap at Gyomu. Just layer those noodles with tomato sauce, cheese, ground beef, spinach, or whatever you want. You can get a 100 yen casserole dish at Daiso.
Va Tout in Roppongi down the road towards the Azabudai/Tokyo Tower side is a French place but their lasagne is quite popular and very reasonably priced at lunch time for the insane volume you get. Nice outdoor seating too for this time of year.
The best I’ve had in Japan so far was at Buttermilk Channel in Harajuku.
Mamma Luisa’s Table (near Shibuya).
Small restaurant that seems to be opened and operated by a single Italian man. The lasagna I had was quite good. Most pasta and preparation seems to be done by him on the day in the restaurant.
CIRO 桜新町店 should have it.
It’s funny how Italian food is generally so common and liked here but so many delicious, key dishes are basically unavailable.
I am still trying to find a place that does handmade ravioli.
Nah, spaghetti or any other kind of noodle pasta is all you get!

I’ve mentioned [La Piccola Tavola](https://www.piccolatavola.jp/) in Eifukucho before for pizza, but their lasagna is pretty darn good too.
Letterpress in Tomigaya does an exceptional lasagna. Exceptional. But only as their lunchbox, and only once a week between Weds to Sat depending on when it is scheduled.

I’d suggest Pancon Yoyogi! Their lasagna was nice!

I just had nice lasagna at a place in Hongo/Yushima area: OSTERIA Cocogoloso. 1,500 yen lunch set; it comes with a decently sized plate of salad and a tea/coffee, and you can make it 大盛り for an addition 150y.

[Il Chianti](http://www.chianti.co.jp/restaurant/index.php) makes a pretty good lasagna IMO. They have locations around Tokyo but I’m not sure if all the menus are the same.
Not a restaurant, but I’ve enjoyed the lasagna I’ve bought at Isetan. There’s usually 2-3 shops selling it on the food floor.
