First Trip to Tokyo/Kyoto Itinerary Check

My family, including my wife, two kids (aged 4 and 9 months), and my parents (Late 60's), will be taking our first family trip, spending 13 days in Japan. We plan to explore Tokyo for 5 days, then head to Kyoto for another 5 days, before returning to Tokyo for 3 days to visit Disneyland and DisneySea. Based on advice from other travelers with kids, we've structured our itinerary to focus on one area per day to accommodate the little ones, allowing us to fully enjoy each location. I’m seeking suggestions for additional activities we could add if we have extra time and the kids are up for it, or if there’s anything in our itinerary we should consider skipping. I’d also appreciate any ideas for things my wife and I can do in the evenings after the kids are asleep, with my parents watching them. Lastly, is it possible to combine or omit any of the temples or shrines in Kyoto to visit Nara Park, or would it be better to stop by Nara Park on our way back to Tokyo?
