Employee Improvement Plan

Hello all,
I recently started a direct hire position and my local government requires every employee to submit a form listing professional goals and plans to reach those goals. My previous private employer had no such thing so I'm at a bit of a loss for what to write. I'm the only ALT here so there's no one I can really compare with.

Has anyone else had to fill out this kind of form? If so, what did you write? Any suggestions would be greatly appreciated.
