Homeroom teachers as third parents?

I’ve heard it mentioned several times that there’s a culture of Japanese homeroom teachers being expected to be “third parents” to their students.

What does this mean, exactly? Counseling them about personal issues? Driving them home after school if need be? I’m just a bit unsure on what that looks like. Thanks for your time!
