Link to official medical certificate form

There's lots of references to a medical certificate being a requirement, but I cannot find an official form. A direct link to it would be greatly appreciated.

Also, does everyone taking prescribed medications need to complete one or is it only when it's listed as narcotic, psychotropic, etc on the controlled substance list? I don't take anything of the completely banned list. I do have a number of annual visits next month specifically for med check ins. I would like to provide them any forms then.
