how to become a certified care worker in japan?

of course i googled it but i dont think i understood or know everything.
i graduated from highschool this year, and i wanted to go to university to move to japan but i have a friend there that i like (not as a friend xd) and want to be together with and i have bpd so i want to go as fast as possible. its really awful for me to not be there right now, especially because of abandonment issues. for him it would also be better because his family is abusive and hes too mentally ill to go to work so he cant move out and hes 19 so he cant get any help for the situation with his family. but if i go there, he can live with me. but i dont know what to do now. as far as i know you earn money when you go to this nursing school and dont have to pay for it. i have enough money for a flight but more than that and my family also is abusive so i wont get any money from them.
another problem is we both are transgender. he has a diagnosis but i dont. if i get one in germany (where i live right now) before i go to japan, do i have to get a new one in japan or can i just continue theraphy there? and if i start taking testosterone here, same question lol can i just continue there or do i need a new diagnosis? sorry that i ask so much i cant really do my own research right now im too worried and excited and whatever idk i cant do it right now
