Hello,
I am a mature student in my final year of medical school and would like to do what is known as a medical elective, in Japan.
Does anyone here know where I might be able to get more information on this topic?
Or maybe you are able to offer advice yourself based on your experience/knowledge?
I would like to go to a big city ideally.

There are a few different options available to you. Different programs have different application processes and acceptance criteria. I would not expect them to be paid.

This blurb from the Yokohama schools page probably applies universally:
