National Health Insurance Proof of Payment

I need to submit proof of payment of my National Health Insurance for the Year-End Tax Adjustment and I cannot figure out which one is okay to use. I already got the one for National Pension. Should I just send the 領収証書? Do I have to go to the city hall to ask for a proof of payment paper that looks like the National Pension one?

  1. Ask your city hall. They’ll be able to tell you what document(s) you need.

    Edit: and say what you need it for.

  2. Year-End Tax Adjustment and Income Tax Return do not require proof of NHI payment (but that of National Pension).

    So I guess your company is requesting it to you for their convenience. I think 領収証書 will suffice but better to ask them.
