No

No. But you should get one.
Your itinerary it the opposite of packed, you can get a couple of extra backup ideas for each day, so you have idea of things to do in case you do everything and wand to do mare after. If aything, some things could be change a bit like Harajuku first (if you want to go to Meiji jingu), then walk to Shibuya, and do Tsukiji Teamlab the next day.
Best way to get to Tokyo to ride the shinkansen will be the Narita Express train.
Suica is not a pass, it is a prepaid card that you can use to pay for your train/subway/bus/tram, as well as pay in vending machine, convenience stores, etc. It is quite useful so yes, would recommend to get one.
