I am not sure if this belongs to /r/translator or not. But, if it does, please let me know, I’ll remove this post and move there for this.
I recently moved to a new place in Japan and Post office has sent a post card to verify my address.
I filled in the name on the form, but I’m not sure about some of the other fields. Google translator calls these fields “Work Name” and “Family name” and “Companion Name”, but none of these make sense to me.
I live alone and don’t have a pet or anything.
I’ve highlighted the confusing fields with blue colour in this image. Could someone please help me what these fields are for?

First one is not relevant unless you are a business.
Second one is names of family members living with you.
Third one is names of people living together with you, other than family.
