Hi I’m travelling from the UK to Japan soon and I have an urgent PCR test booked the day before my flight. I understand that Japan wants this supplementary form filled out:

It seems like it’s changed over it’s lifetime though and in its current form, I don’t see any requirement for a stamp/signature, whereas all discussions online mention that this is mandatory except in a few countries.
Is the stamp/signature still required?
Also, I’m using [expresstest.co.uk](https://www.expresstest.co.uk/). Has anyone used them recently to get into Japan?
