Does it have the “bad rep” that rap music used to have in the US ? I see Japan rappers smoking marijuana and stuff on their clips, knowing this is super illegal, how do the public react ?

These days rap is usually seen on YouTube, but some people still watch MTV
The public watching are teenagers who probably get up to worse themselves. No one else cares.
The brand of rap that’s associated with drugs is not mainstream, but can easily be found on the internet so I guess it does have its followers. You sometime hear about some rap artist getting arrested on the drug charges so people just associate it with bad life choices.
These days when someone mentions rap it’s the rap part that’s in a J-Pop or K-Pop song
Not familiar with the current scene, but there is some good old school 90s rap out there. For example: https://youtu.be/pxsx4027djE
