Hi everyone. I’m going to Japan at the end of this month and I need to bring my medication,but since it’s gonna be over a month I need to get the Yunyu kakunin-sho and I’m pretty confused on what to do. Do I just send them pics of the bottles with my prescriptions or should I also get a signed letter from my Doctor saying what I take,how many and all that?
If you’ve done this and can help me I’ll be very thankful.

I filled out and submitted mine for my diabetes supplies. The link is below. It includes examples. I printed the pages out, filled them out, scanned them back in and emailed these 3 things: that form(1), prescriptions for each drug/item*(2), and my itinerary to the airport I’m landing at(3). They responded within a day or 2 with the form filled in and stamped by them. I printed that and put it in a folder along with a prescription for each drug/item within the last 6 months that will accompany the stack of meds.
*I had to ask my Dr. for paper prescriptions cause I didn’t have any at the time.
