I personally like Yonka (Omotesando), Aveda Spa (Omotesando), Thann (in InterCon in Akasaka or Park Hotel Tokyo in Shiodome), and Spa Damai (Daikanyama).
Hotel Spas are also nice, but double the price of the spas mentioned above (for some reason, the Thann ones are reasonable despite being located in hotel).
