I suffer bad acne periodically on some body parts especially around underwear covered area. I have gotten used to it but the scar is getting worse especially in winter when it’s dry.
I heard that products containing benzoyl peroxide could be useful. Looking for OTC product, body wash, or any home treatments that could help with body acne like this.
Also, does dermatologist generally accept insurance for treating acne problems?

Go see a dermatologist. They can prescribe something that is covered by the insurance and that will in all likelihood be cheaper than OTC stuff. Don’t try to self medicate around your underwear area.
Getting something checked out is usually covered by insurance. I’m from America so I deeply understand the hesitancy to get it checked out if there is any, but even things that aren’t covered by insurance will be in the 20-30,000 yen range. Healthcare is accessible and affordable in Japan.
Go to a doctor and see what they recommend.
