Just a few thoughts:
Day 10 and 11 : You could not go back to Tokyo, leave Yokohama for another trip and head West earlier.
Also, with a little bit of adjustment, you could get away with a 14 day JR pass (e.g day 10-23).
The pass is wasted in Tokyo and Osaka/Kyoto/Nara as local transport is cheap.
Rather than go from Hiroshima to Koyasan (a long trip plus walking, then long trip next day again), you could go from Hakone (Odawara) straight to Hiroshima and work your way backwards so that you leave from Osaka for Koyasan.
