Sunnyside is a good bakery chain (at least in Osaka). Good shokupan and decent sandwiches (for Japan), and they often have rolls you can use for hotdogs and hamburgers. Their fresh curry pan are fantastic!
What kind of bread? What’s your definition of good?
As for me, I mainly consume sliced bread and occasionally croissants; and good bread is made without margarine or shortening, just butter. It’s very hard to find this, so I already gave up.
I’m health-conscious, but also cheap, so I buy my bread from OK Store’s bakery (which uses shortening and margarine) and also Pasco’s sliced bread which comes in a blue bag.

A small bakery chain called Sumomo. They have a few shops scattered around Kyushu. The one near us has some of my favorites, like curry pan, French toast, tamago pan, and other assorted breads, muffins and sandwiches.
City Bakery. They have amazing scones (incl a bacon maple scone), and a salty pretzel croissant. Not so much of that marshmellow soft type of bread.
Edit: Another good one is Sawamura Bakery in Shinjuku’s Newoman area. Fanatastic crusts on their bread.
Local couple making sourdough in my neighbourhood down in Kyushu is great. If you’re in Tokyo there’s also a great Danish bakery called brod in Hiroo.
Anyone living near Uji Kyoto ? Can’t recommend enough “Tamaki-tei”.
For really good baguettes, fluffy and soft inside with a thin, crunchy crust:

So far I haven’t found baguettes as good as in these two places despite having tried all famous bakeries in Kobe.
I’ve got a もしかして宇宙人 (moshikashite uchuujin) close to me and it’s becoming very dangerous.
I like doughnuts from Misaki Donuts. It’s in Yokohama. Also, I like chocolate croissants from the city bakery.

DANKE in Shizuoka makes a German-style bread that is the exact opposite of normal Japanese white bread. Ever couple of months I place a big order of various breads from DANKE and just freeze it until I need it. Great stuff.

Hatsu Tatsu in Osaka is my go-to for baguettes and other hard breads.
