Injury and Sick Allowance- Employee Health Insurance

I was searching for details on the internet about the Injury and Sick Allowance application procedure at the Japan Health Insurance Association (Kyoukai Kenpo) but I found some conflicting information or I couldn’t find more details or where I could have more detailed information. I asked at a Employment Agency and City Hall but they don’t have much information. For example:

– I have paid the Japan Health Insurance Association for years, but in different companies. I’m working for this company for 6 months now. The time I have worked in other companies and paid the employment insurance counts for the 12 months of continuous employment mentioned in the Allowance application? Is it ok have a gap between jobs?
– Regarding the salary calculation, the Kyoukai Kenpo website informs that the allowance calculation is the Average Standard Salary of the previous months divided by 2/3. In the website they inform that the employee “average standard salary” is the total amount of money received before taxes and the monthly premium is calculated according to this amount. Because I receive per hour plus allowances like special hour allowance or extra hour allowance, I checked the forms and it seems they calculate the average salary according to the basic hour salary for 8 hours and exclude the allowances. How it works? Specially if I have been paying the premium according to the total amount I received which is the “average standard salary”. I can’t understand.
– About the Allowance disqualification in case of retirement or if the person lose the job during sick leave and have less than 12 months of continuous employment, so in this case the person can’t continue receiving the allowance? Again the 12 months of continuous employment thing. I can’t understand.
