Statement of Physician Question

Hello! I’ve seen some people with similar questions but here’s mine. I was diagnosed by a counselor back in 2014 with social and general anxiety. I saw her for about a year when I was finishing up my associates degree. I applied for the JET program in 2017 and had my regular physician fill out the form. She gave me a prescription for anxiety meds at the time (which i never took) just in case i needed them abroad. I also applied last year and had a walk-in clinic fill it out as my old doctor no longer practiced family medicine.

This year I got a new job last month unexpectedly ( I applied in March and was called in late September about the job). Then I had to travel for the new job the last two weeks.

All that is to say. I’ve not had any major issues with anxiety since like 2017? I don’t have a regular doctor anymore, I don’t have insurance to find a new doctor since my new job’s benefits dont kick in until like January for me, and I’m just wondering if I can still apply to the JET program without listing anxiety on my application? Or is there another option I’m just not seeing?

PS. I did also contact my old counselor to see if I could just pay out of pocket to cover the session if she could fill out the form but she said that she would not feel comfortable doing so since I hadn’t seen her in like seven years. Neither of the doctors i saw before practice family medicine anymore either.
