statement of physician question

hey, so i’ve recently been diagnosed with specific learning difficulties associated with ADD, dyslexia and dyspraxia but don’t get any treatment for them and therefore don’t have a GP or qualified doctor that can fill out the form. Has anyone else been in a similar situation?

I’ve been advised by the psychologist that diagnosed me to attach my diagnosis as proof but I’m not sure if that will work…

  1. Your psychologist is not a medically qualified doctor (unless he is a psychiatrist), therefore his or her signature on the form is not acceptable. You need to go to a GP or private practice and ask for someone to corroborate this diagnosis and write it down in your medical report, even if you do not take any medication. They are the only ones who can legally sign this document.
