I am currently trying to figure out how to get a Yunyu Kakunin-sho form. I am applying for a language school through Go Go and they said I need to fill out their form in order to bring in my Famotidine 20MG. They said they can’t help me fill it out or get it.
I tried looking online for the form or how to get one but I’m getting directed to a website called JET? Which is a exchange program?
How do I get the form? Japanese embassy?
What documents do I need for this?
Also would I have to include tums into this documents? Apologize if it’s a stupid question.

I filled out and submitted mine for my diabetes supplies. The link is below. It includes examples. I printed the pages out, filled them out, scanned them back in and emailed that form, prescriptions for each drug/item, and my itinerary to the airport I’m landing at. They responded within a day or 2 with the form filled in and stamped by them. I printed that, along with a prescription for each drug/item within the last 6 months all in a folder that will accompany the stack of meds.
