Quick Physicians’ form question.

I take Lisinopril for my high blood pressure, and it’s something i’ve been taking for years, probably since 2015. I still take it to this day. Just one in the morning and that’s it. I have gone days without taking it, sometimes weeks. (I just forget.)

Do I still need to call a doctor up for a the statement of physician form? I feel like it’s not that serious, but I don’t want to assume and fuck up my application.

  1. If you take a medication regularly for something, you should list it in the Statement of Physician.

    Also, please take your BP meds as prescribed, so if your doctor says to take it every day, please don’t skip it.
