I had UTI before and tested positive (asymptomatic) to COVID last year. Do I have to include that on Self Report of Medical Conditions? Also, do I need to ask for a Physician’s Form for these?

The form is asking about medical conditions and not about one-off medical history happening. You don’t have to write when you got chicken pox or when you sprained your ankle, either.
