I didn’t know I was supposed to submit the form at the checkpoint at the airport, and when I got through to the information center, I asked about it, and the staff called someone and told me that I couldn’t go back through to apply for it and would have to apply somewhere else (I don’t remember where or what they said). I live in Shinjuku, so can I apply for the work permit at the Shinjuku city office (where I registered my address and applied for medical insurance), or do I apply for it somewhere else?

The work permit is not a city function, so no.
I would assume you’ll need to go to your local immigration bureau
If you live in Shinjuku, the closest immigration bureau is near Shinagawa Station. You can get a bus from the East exit or else you can hoof it in about a half hour.
