I have cultural activities visa here in Japan. For my 1 year fellowship in a Japanese Hospital(but they dont give me any stipend or salary). What is the procedure I need to do when I leave Japan on August 25,2023 and planning to return here on the 31st of August. So far I heard that I need to fill up something with the Japanese Immigration before I leave so that when I get back ill just show my Zairyu Card

Due to corona I’m not up to date what it’s called though.
You will need to fill up the special re entry permit at the airport, there’s a section in the embarkation card for that purpose. Then, the immigration officer will staple it to your passport.
