Need help with Yunyu Kakunin-sho

Hey! I will be studying in Japan for 1 year as an exchange student.
As I have to bring a lot of medication with me, I was told to fill out the Yunyu Kakunin-sho.
However, I’ve run into some problems:

FORM 12:

– I have run out of space to write down my meds in the first box, so I added an extra (blank) sheet with the info required – but I don’t know if using a blank page is ok or how else to do it?

– There is a box “Name of manufacturer…”, but there is absolutely no way for me to fit everything in that small box, as I had to add 12 medications which are mostly from different ones…?

FORM 13:

– I’m really confused about the “efficacy” box. I plain out just googled the meds and wrote down descriptions off wikipedia.

I have looked at the example sheets and googled, but I still don’t really know what to do about it. Can somebody – maybe someone who had to fill it out themself – help me?

  3. Fill out the online form! You won’t run into issues with running out of space. Although be careful to save your progress or try to complete all the boxes in one sitting.     Also, for the efficacy, I just typed down the intended effect of my medication. Ex: To lower blood pressure related to x diagnosis. If you have a doctor’s note about your meds, it really helps. I was really unsure of what I was typing but it was all accepted in the end so don’t stress too hard. They are also good about sending emails if anything is needed.
