Generally an orthopedic clinic, 整形外科. There’s many around, but if English speaking, something like Tokyo Sports & Orthopaedic Clinic or Dr. Kakuko Sports Clinic. I’ve never been to either since I can speak Japanese and multiple tests have shown I just have bad posture, but I’ve heard general good things about both. Maybe someone else will have recommendations for lower back pain specifically.
Kameda Kyobashi Clinic was pretty good for my leg issues, they do spinal / head and neck / sports injuries. English speaking staff if you need it.
I can vouch for Kakuko. Takes NHI and has English speaking do tors and therapists. But it’s small (no MRIs). If near Shinagawa, best go to Inannmi as that’s large, does surgery, and basically covers all bases. Higashi-Shinagawa.
I can also recommend 御殿山整形外科リハビリテーションクリニック
it’s in Osaki. The head doctor there speaks very good English and is really positive and supportive. He enjoys the chance to use his English and gives you the time needed – not just in and out. I did PRP therapy there for a shoulder injury. Recommend. I don’t think they have in site MRIs but easily arranged.
For a big hospital with all on site, then 稲波脊椎関節病院 (Inanami) is the way to go.
